DUE TO COVID-19, OUR OFFICE HAS LIMITED APPOINTMENT TIMES FOR THE SAFETY OF OUR PATIENTS AND STAFF. A $45 FEE WILL BE COLLECTED AT THE TIME OF SCHEDULING. THIS FEE WILL BE RETURNED AT THE TIME OF YOUR APPOINTMENT. A 24 HOUR NOTICE IS REQUIRED FOR RESCHEDULING OR CANCELLATION.
