We are happy to schedule TWO family members together. ( Max of 2 per day) This submission is a REQUEST to schedule the appointment, but NOT automatically booking this appointment. You will receive a notification once the appointment is entered within our system. Complete insurance information is needed and validated to book through this system. Requests with incomplete insurance information are not able to be accepted. Please include the latest ID #'s and primary insured's information to ensure correct claim filing and maximize your benefit. Please CALL to CANCEL/ RESCHEDULE appointments, to ensure the correct TYPE of appointment is scheduled.
